Hospice Care Directory
Hospice care faciilities and agencies provide medical and palliative care for people with terminal illness, generally within the last 6 months of life. Hospice facilites can be independent or located within hospitals. Hospice care is also provided increasingly in the home.
Hospice care is different from other health care in several aspects:
Team of doctors, nurses, and other health care workers including social workers, counselors, clergy, home health aides, clergy, volunteers and therapists
Recognizes the spiritual needs of patients and families
Provides care in the home as well as in-patient care
Respite care: Hospice service provides caregivers and family a much needed break from intensive care through respite care.
Conferences of family are scheduled regularly
Bereavement care: Following death, the hospice care team gives help with the grieving process to surviving family members
